Teacher Assistant Application Form
After downloading and completing the application form and background check consent form, print a copy for your records and a copy to submit for employment consideration. Sign and date the forms, attach your resume, an official copy of all college transcripts, and a copy of your Oklahoma Teaching Certificate.
For applications to remain active they must be renewed each year.
